NATIONAL CRIMINAL BACKGROUND CHECK AUTHORIZATION
I authorize Star Coaches, Inc. to perform a complete National Criminal Background Check at any time prior to, or during my term, as an Entertainer Coach Driver for Star Coaches, Inc.
I understand that it is my responsibility to provide Star Coaches, Inc. with a National Criminal report from a law enforcement agency in order to be considered for hire as an Entertainer Coach Driver operating equipment belonging to Star Coaches, Inc.
Furthermore, I authorize representatives of Star Coaches, Inc. to discuss with Canadian immigration officials any information contained within my drivers file, but not limited to any law enforcement criminal database reports.

I understand that information I provide regarding current and / or previous employers may be used, and those employer(s) will be contacted, for the purpose of investigating my safety performance history as required by 49 CFR 391.23(d) and (e).
I understand that I have a right to:
a) Review information provided by a previous employer; and
b) Have errors in the information corrected by previous employers and for those previous employers to re-send the corrected information to the prospective employer; and
c) Have a rebuttal statement attached to the alleged erroneous information, if the previous employer(s) and I cannot agree on the accuracy of the information.
